We may deny your request to inspect and copy in certain circumstances. If you are denied access to protected health information, you may request that the denial be reviewed. Another licensed health care professional chosen by Med Harmony will review your request and the denial. The person conducting the review will not be the person who denied your request. We will comply with the outcome of the review.
Right to an Electronic Copy of Electronic Medical Records: If your protected health information is maintained in an electronic format(known as an electronic medical record or an electronic health record), you have the right to request that an electronic copy of your record be given to you or transmitted to another individual or entity.

If you request this accounting more than once in a 12 month period we may charge you a reasonable, cost-based fee for responding to these additional requests.
